We need solutions that work, in practice – globally.
New Zealand has proven that elimination is possible, with cooperative self isolation and minimal policing. But we can only maintain our virus free status with strict border controls, and two week quarantine at the border. That works for us, but doesn’t solve everyone else’s problem.
Two major aspects to this – preventing transmission, and improving recovery.
On the improving recovery front, our existing systems are not working. Double blind trials are not an appropriate response for this sort of situation. What is required is a form of both trust and big data.
We give every doctor the complete freedom and responsibility to do whatever seems to them to be the best thing for that particular patient in that particular context, we require them to make short but thorough notes, and we look very closely at what actually happens. We inform everyone of approaches that are demonstrated to work, and we focus about half of the research attention on those areas.
I strongly suspect diet will be very important – with high dose vitamin C (over 10g per day, in at least 2 doses of at least 5g per dose per day) combined with monitoring vitamin D and zinc levels will important factors.
On the prevention front, vertical separation, with full head covering masks that have an inlet with a small battery powered fan above the head and something like an elephant’s trunk taking exhaust air to the floor (with velcro connections at waste knee and ankle) should significantly reduce spread, if combined with hand sanitation (hand sanitiser available on both sides of every door, and at every keypad).
If these are deployed, then the pandemic can be eliminated.
If these are not deployed, then there must be some other reason for the pandemic, and the conspiracy folks have gotten something right.
